Insurance
Waiver for
students who bring their own insurance Students
who show proof of that they already have accident and medical insurance, (equivalent
to the coverage required by Oregon state law), can fill out a Waiver Form
and do not have to pay for Extended Coverage Accident and Medical Insurance. In
order to waive the extended coverage health insurance fee you must bring PROOF,
written in English, that you already have accident and medical insurance coverage
that meets the following (Oregon State Law) requirements: -
Medical benefits of at least $50,000 per accident or illness, or $250,000 major
medical coverage,

A deductible not to exceed $500 per accident or illness

Repatriation of remains of at least $7500
-
Medical evacuation to the insured person's home country of at least $10,000
-
Funds must be payable in the USA
Your
proof of insurance must also show: -
Dates of coverage (begin date and expiration date)
Most
universities require students to carry insurance coverage from the first day of
the term, lasting through the first day of the next term. If your
insurance expires between the last day of the term and the first day of the new
term, you will be required to buy additional insurance. -
Student's name as the insured

telephone number for the insurance company

Documentation written in English
If
your insurance does NOT cover the amount listed above, in most cases you will
be required to purchase Extended Accident and Medical Insurance through your respective
OUS school. Insurance
Waiver Forms must be filled out within the first 20 days of start of the term.
Each university handles these forms differently. In some universities this form
is available in the International Student Office, in other schools this form is
handled by the Business/Cashiers Office or Student Health Center. You
will learn more about this during the International Student Orientation held at
each campus. If you have questions, please ask your international student advisor. Know
about your insurance plan It is your responsibility to confirm the details
of your policy with your insurance company. If your coverage is through a local
or American insurance company, it may not cover medical evacuation or repatriation
of remains. You should double-check this. If your insurance does not cover these
items, you will need to purchase an additional policy or buy the extended coverage
policy. For
